Insurance Information

At Wedgewood Family Practice,
we specialize in you, our patient.

When you visit the office, please check in with the receptionist. Please inform the staff of any changes in your address, phone number or insurance coverage. We have provided our forms online for your convenience (PDFs). You may print and fill them out to bring with you.

Please be prepared at each visit to inform us of all the medications you are taking. This is important because, if you are seeing any other doctors or specialists that may have prescribed medications for you, we will not have a record of it in your medical chart here at Wedgewood. To safeguard against the possibility of drug interaction, please always inform us about what medications you are currently taking (prescription and over–the-counter).

Lab Work and X-rays
We have lab and X-ray facilities at both the Suncrest and Westover locations for your convenience. Usually, we provide lab results to you within 10-14 days following your appointment. Please call the office to schedule an appointment if you need lab work.

Medical Questions
If you have questions for your doctor or nurse between office visits, please call the office. The receptionist will be happy to take a message and the doctor or nurse will return the call as soon as possible.

Insurance and Billing
We participate in many insurance plans and will bill your insurance company for you. Our insurance and billing department number is 304-598-2233.

For Emergencies
If you should be faced with an emergency when both offices are closed, you can go directly to the Monongalia General Hospital Emergency Department. If you have an urgent matter that you need to discuss with a physician, please call our answering service at 304-598-1212. The service will connect you with the physician-on-call for Wedgewood Family Practice. Some insurance carriers require notification of your Primary Care Physician prior to your visit to the Emergency Department. Please be aware of your insurance carrier’s requirements.

Prescription Refills
If you need any of your non-narcotic prescriptions refilled, please let us know at the time of your office visit. If you need a refill between visits, call your pharmacy directly and they will fax the order to us for approval. For narcotic prescription refills, please contact our office. Please allow 48 hours for all refill requests to be processed.

Psychiatry/Behavioral Health Services
We offer a comprehensive array of behavioral health treatment services to children, adolescents, adults, and the elderly in the following areas:
